About the Organization
The HealthPath Foundation of Ohio (HPF-Ohio) is a supporting organization of The Greater Cincinnati Foundation. A grantmaking organization dedicated to improving the health of underserved Ohioans, HPF-Ohio has historically awarded prevention-focused grants to nonprofit and government organizations within its 36-county service area.

Job Description
HPF-Ohio is recruiting a health and human services professional for a ten-month contract period to provide leadership and oversight to all aspects of the Foundation's Community Connections Pilot Project (CCPP). The Community Connections program consultant will work cooperatively with Foundation staff and board to plan, implement and evaluate the pilot project. 

The desired short term outcomes of the CCPP include broadening the geographic reach of the Foundation in its service area; increasing the number, distribution and diversity of Foundation volunteers; and dissemination of best and promising practices in family violence prevention. The Pilot Project will focus on funding grants of up to $5000 to support family violence prevention programs in the Foundation's service area. 

Key deliverables will include the following: 
  1. Develop grant application materials; 
  2. Plan, organize and conduct information and training sessions for potential applicants; 
  3. Recruit and train volunteer grant readers and facilitate grant review process; 
  4. Develop evaluation plan, including data collection tools. 
Qualifications 
Due to the nature of the position, previous grantmaking experience and knowledge of health and human service related issues are required. The ideal candidate will have a minimum of a bachelor's degree in a health or human services related descipline; previous experience in program development, implmentation, management and outcomes measurement; three or more years in grant making; and excellent interpersonal, presentation, verbal and written communication skills; a commitment to flexibility and inclusion; and compassion for the underserved in the community.
